Otoplasty may be performed in children or adults. In children, surgery is commonly undertaken between five and six years of age, when the ears are largely developed and before starting school, although many patients choose to undergo surgery later in childhood or adulthood.
The procedure typically takes one to two hours, depending on the complexity and whether one or both ears are treated. Otoplasty is usually performed as day surgery, with patients returning home the same day.
Prominent ears usually result from an underdeveloped natural fold of the ear, excess cartilage causing the ear to project, or a combination of both.

Surgery is performed through an incision behind the ear, allowing access to the cartilage without visible scarring from the front. The cartilage is reshaped and repositioned using a combination of sutures and contouring techniques to achieve a more natural ear position.
Throughout the procedure, care is taken to maintain the natural contours of the ear while avoiding an overcorrected appearance.
Following surgery, a protective head dressing is applied and worn for the first few days. This is replaced with a soft headband, which is typically worn at night for several weeks to protect the ears during healing.
Swelling and bruising are expected and vary between individuals. Discomfort is usually mild to moderate and well controlled with simple pain relief.
Most patients return to school or work within one to two weeks and resume non-contact activities shortly afterwards.
Final ear shape continues to refine over several months as swelling resolves and the tissues settle.
